    $25.00 FEE WILL BE CHARGED ON RETURNED CHECKS TO COVER BANK CHARGES

    By my signature, I submit that I will not hold the officers, coaches, or officials of the Meadowlake Youth Baseball League or the Richland County Parks & Recreation Department liable for any injury to or accident involving my child/children while participating in this youth sports program. Registration fee includes medical insurance. I also agree to assist in the concession stand or find suitable replacements on my assigned day(s) during the spring and summer of the 2026 season.
